BABINSKI

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client Supine

Lightly run a sharp object along the bottom of the foot
Big toe extends. & the other toes abduct. Upper Motor Neuron Lesion

(A positive test is normal for infants up to a few weeks old)

VALSALVA

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client Seated

Ask client to take a deep breath and then "bear down"
(as if forcing a bowel movement)
Increased pain. Space Occupying Lesion.

(such as a herniated disk, a tumor, or osteophytes)

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client supine

Gently compress the jugular veins for ten seconds, then ask client to cough
Pain in lower back region Intrathecal Spinal Cord Pressure

(Theca is the covering around the spinal cord… i.e. pia, arachnoid, & dura mater)

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client standing

Have client close eyes for 20-30 seconds
Client sways excessively or loses balance. Upper Motor Neuron Lesion

SLUMP

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client Seated Upright
(hands behind back, legs dangling).

1) Flex torso (w/head supported)
2) Apply pressure on clients shoulder
3) Client flexes head
4) Apply pressure on clients head
5) Extend knee & dorsiflex foot
6) Extend head
Unable to do all motions

or

Decreased symptoms with head extension
Neuromeningeal Tract Lesion

STRAIGHT LEG RAISE

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client Supine

Raise client’s leg with knee extended to point of pain.
Slowly lower leg to point of no pain.
Flex clients neck or dorsiflex foot.
Pain with neck flexion or foot dorsiflexion Neurological Lesion

WELL LEG RAISE

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client Supine

Raise client’s leg (non-test side) with knee extended
Pain in the opposite leg Herniated Disc

BRUDZINSKI-KERNIG

HOW TO PERFORM TEST

POSITIVE SIGN

INDICATION

Client supine
(hands behind head)

1) Client actively flexes the neck
2) Client actively flexes the hip to the point of pain
2) Client flexes knee
Pain disappears with knee bending Meningeal Irritation
